 Dear Band Parents,

Many of you may already be aware that there have been several changes made to the EASD Clearance Policy.  ALL band chaperones, and any parent volunteer who has extended contact with students must have the following clearances:

1.       PA State Police Criminal Background Check  (form SP-164) ($10)

2.       PA Child Abuse History Clearance (form CY-113) ($10)

3.       FBI Clearance (requires fingerprinting) ($36)

4.       TB Test

Once you have all four clearances, you do not have to renew them each year provided that you remain an active volunteer within the EBPA. In addition to the four clearances listed above, you must also complete and submit the following:

1.       Volunteer/Chaperone Application Form

2.       Volunteer/Chaperone Disclosure Form

You must also read the Volunteer/Chaperone Manual.

You can find the paperwork and/or apply for these various clearances online:

•   www.pa.cogentid.com//indes_dpw.htm   ~  is for the FBI fingerprinting. You can apply online as well as get detailed information regarding the application process.

•     TB Test  

The clearances can take several weeks to be processed, so if you plan to volunteer with the band over the summer (band camp) or during the fall season, please consider starting the process now.

The results from the Criminal Background Check and Child Abuse Check will be mailed to you. Once you receive your clearances, have the results from the TB test, and have completed the other district forms, please make a photocopy of all forms and turn them in to Mr. Kimmel. He keeps a mailbox at every elementary school, the EIS, and the high school.

After having your fingerprints taken, please notify Mr. Kimmel. He will ask you for the registration number on the paperwork you received during the process.  Mr. Kimmel can be reached at 721-1478 ext. 15211 or j_kimmel@easdpa.org.
